Dear Chili

If it's for a school child (pupil): How old is your friend?
Contact the school you would like for an exchange directly. As far as I know, the USA and Canada shouldn't be a problem.
There are many exchange travel agents for high school students. Contact them and chose a school, then contact the school for accessibility details of the buildings.

If it's for a student: It's worth the time to look through the HEAG: http://www.european-agency.org/agency-projects/heag
We would even have an article about that in English, but as that one doesnt work at the moment for some reasons, I give to the link to the German one as I assume that you speak German: http://www.myhandicap.de/studium-behindert-heag-uni.html?PHPSESSID=0efc0bf3c7d3aadb69b83da14835610b

Hope to have helped you!
